I am a beginner, learning about chakras and crystals. However, I find a lot of conflicting accounts of which crystals/stones go with which chakra. Is there a definitive guide? "Who" decided which crystal go with which?

Thank you for your questions.
The reason one can find a lot of different information on crystals for chakras is that there are actually many crystals that work nicely with different chakras. Human beings are very complex and in interaction with crystals we all can receive different things. This is why there isn't -- and will never be -- one definitive guide for everyone.
That, however, is also the beauty of working with crystals. We need to pay attention, develop our own way of discovering how the energy affects us, and be patient and consistent learners as we develop our sensitivity, our intuitive guidance, and skill of working with crystals for our benefit.
All that being said, for more than two thousands of years people have been using crystals for healing purposes and writing about them. Many of these written accounts are still in existence, and many crystal book authors built upon that knowledge while also using their own connection with crystals as guidance. Due to their chemical composition and crystalline structure, it has been determined over time that some crystals are particularly useful for certain things, and this is also why there is so much similarity between many books.
It has, however, become obvious in crystal circles that no book is going to be able to replace direct experience and personal learning.
